The Science of Hot Stone Therapy:
Hot stone therapy uses heated stones (often basalt) placed on or moved over the body, usually combined with massage, to relieve pain, reduce anxiety, and improve sleep. Research is still limited but growing, with several randomised trials and case reports.
Mechanisms and Treatment Methods
Hot stone massage combines thermotherapy + mechanical pressure + massage. Stones heated to about 45–55 °C are pressed and moved over muscles for ~15 minutes, followed by manual massage of specific back and shoulder muscles (Li et al., 2023; Febrianto et al., 2023; Zhang et al., 2023; Ansari et al., 2024).
Some protocols place stationary hot stones on chakra points over a towel (Zhang et al., 2023)or on the lower abdomen (CV4) (Mironnet & Monpetit, 2015). Traditional explanations include “warming meridians” and stimulating acupoints, while modern trials also measure changes in deep muscle thickness by ultrasound as a possible pain mechanism (Febrianto et al., 2023).
Typical Applications and Outcomes
Chronic back / low back pain: Pain scores, function, daily activities (Li et al., 2023; Li et al., 2025; Febrianto et al., 2023; Srisukho & Chinthakanan, 2017; Yang et al., 2018; Du et al., 2022)
Postoperative orthopedic patients: Anxiety, sleep quality(Ghavami et al., 2019; Kim et al., 2018)
Hemodialysis patients: Sleep quality (PSQI)(Zhang et al., 2023)
Advanced cancer (palliative care): Multiple symptoms (pain, anxiety, wellbeing)(Li et al., 2024)
Effectiveness and Clinical Evidence
Pain relief:
Randomised trials in chronic back pain show hot stone massage reduces pain (Global Pain Scale, NRS) more than a topical NSAID plaster after 2 weeks, with additional gains in pain self‑efficacy and mental quality of life (Febrianto et al., 2023).
Reviews and small clinical studies in low back pain also report significant pain reduction, often with temperatures >38 °C and p < 0.05 (Li et al., 2025; Du et al., 2022). Case‑based nursing reports using heated Bian-stone plus herbal penetration describe relief of low back pain and improved daily activities (Radziejowski, 2018; Yang et al., 2018).
Sleep and anxiety:
In orthopedic surgery patients, hot stone or slow-stroke back massage improved sleep quality and reduced postoperative anxiety versus routine care (Ghavami et al., 2019; Kim et al., 2018). In hemodialysis patients, 12 sessions of basalt hot stone massage reduced PSQI scores (better sleep) compared with controls (P < 0.001) (Zhang et al., 2023).
Palliative cancer care
In advanced cancer inpatients, single-session hot stone massage reduced pain, anxiety, depression and improved wellbeing in a high proportion of massages, with 100% patient satisfaction (Li et al., 2024).
Safety and Risks
Most clinical trials report hot stone therapy as generally safe, with only mild, transient skin pain or irritation when temperature is adjusted (Febrianto et al., 2023; Zhang et al., 2023; Barriga & Gabriela, 2017).
However, case reports show serious burns can occur when stones are too hot or applied directly to vulnerable tissue, including a thermal burn and infected ulcer on a prolapsed uterus after self‑applied hot stone treatment (Trulock, 2008). Controlled studies therefore limit stone temperature to about 43–50 °C and often use towels as barriers (Li et al., 2023; Febrianto et al., 2023; Zhang et al., 2023; Mironnet & Monpetit, 2015).
Conclusion
Hot stone therapy, usually combined with massage, has emerging evidence for reducing chronic musculoskeletal pain, improving sleep, and easing anxiety and cancer-related symptoms, with mostly mild side effects when delivered in standardised, temperature‑controlled ways. The science is still limited by small sample sizes and heterogeneous methods, and improper use can cause serious burns. When applied by trained practitioners with careful temperature control and skin protection, it appears to be a promising non‑drug complementary therapy for pain and stress‑related problems.
